Bereavement Services/Grief Counseling

As part of a well developed hospice program, Hospices must provide bereavement support services and counseling to all family members for up to one year after the death of the hospice patient. This includes children as well as parents, spouses as well as siblings.

Our agency helps develop a bereavement program that could either be maintained by your MSW or assigned Bereavement coordinator. Nonetheless, we have a team of MSWs as well as experienced Bereavement Coordinators that are readily available to help our agencies with Bereavement Services.

The Services we offer include:

-Bereavement coordination-monthly follow ups/reports

-Initial Bereavement Risk assessment visits or phone follow ups

-Post death bereavement risk assessment visits or phone follow ups

-Monthly continued bereavement support through mailings, phone follow ups or support groups

-Grief Counseling with experienced counselors for patients who are at high risk of bereavement complications

-Documenting and reporting all bereavement related communications and assessments
